The Merton Transition Team support young people between 14 and 25 years of age with severe and complex needs and disabilities making the transition from childhood to adulthood.
The team work directly with the young person and their family/carers to ensure that their views, wishes and feelings are central in the planning of their future life as an adult. The Merton Transition Team will ensure that parents/carers are involved in every step of the process by sharing knowledge and information, enabling them, along with their child, to make informed decisions about their child's future.
- Support the young person and their parents/carers.

- Provide the right information at the right time.
- Make sure that the young person and their parents'/carers' views are heard.

In order to receive support from the Merton Transition Team, the young person will need to be a resident of the London Borough of Merton and meet eligibility criteria for either children's or adult social care, dependent on age (14 to 18 for children's social care, 18 to 25 for adult social care).
